Credentials verified before you go live

Patient safety comes first - so we verify every clinician.

Patients searching CHMD for a provider deserve to know the credentials they see are real. That’s why every Licensed Provider application is reviewed personally by CHMD staff. Your NPI, state license, and DEA (if you prescribe) are all verified before your profile is visible to a single patient.

MD Network Directory

Be found by patients searching for a trusted provider.

Your verified profile lives in the MD Network - where patients search for clinicians by specialty, state, and whether you offer telemedicine. It’s the opposite of a crowded review site: every clinician listed is credential-verified, your badge is public, and you control whether you’re discoverable.

  • Patients find you by specialty, state, and telemedicine availability
  • Your verified-credentials badge is visible on every listing
  • Pause discovery or stop accepting new referrals any time - one click, reversible
  • Practice owners and staff providers are both discoverable, with clear affiliations

Appointments

One calendar for telemedicine and in-person visits.

Whether patients see you in the office or over video, everything flows through the same calendar. They fill out a proper intake when they book - demographics, insurance, measurements, history - so by the time you see them, you’re ahead of the conversation instead of catching up.

  • Video visits run on Daily.co - one secure meeting link per appointment, nothing to install
  • Track every visit from scheduled through completion - signed, time-stamped, done
  • Ask for more info before the visit if something’s missing from intake
  • Confirmations and reminders go out by email and text automatically - no-show rates drop
  • Real-time alerts on new bookings, confirmations, and cancellations - nothing slips by
  • Attendance tracking with your signed attestation on every completed visit

Insurance intake, verification, and per-location acceptance

Insurance handled the way you'd actually want it.

Patients submit insurance during booking. You and your staff work through a clean verification queue - per-location insurance acceptance, carrier rosters, and verification statuses keep the real-world mess contained.

Complete Intake At Booking

Patients enter their full insurance details and upload photos of their card right when they book - no back-and-forth emails, no fax machines, no missing info at check-in.

A Queue You Can Actually Work

Every submitted insurance sits in a verification queue with a clear status at a glance. Work through them at your pace, mark each one done, and never wonder what’s still pending.

Different Carriers, Different Offices

Your north office takes Blue Cross. The south office doesn’t. Configure which carriers each location accepts so patients only book where their plan works. Self-pay is always an option.

Multi-Location Practice

Every office, on its own page.

Running one practice out of three offices shouldn’t feel like running three websites. Each of your locations gets its own page - with hours, phone, insurance accepted, parking notes, and patient reviews that roll up to your overall reputation. Patients book into the office that works for them; you see what’s booked everywhere at a glance.

  • Unlimited locations per account, each with its own page and photos
  • Weekly hours plus one-off exceptions for holidays, closures, or special hours
  • Per-location phone, email, licenses, and external links
  • Accessibility info patients actually need: wheelchair access, elevator, parking
  • Turn telemedicine on or off per location depending on what each office does
  • Assign which providers take appointments where
  • Location-level reviews roll up into your practice-wide rating

Practice Staff & Team

Bring your whole practice under one account.

If you own the practice, you can invite other providers to work under your account - whether they’re clinicians you’re hiring onto your staff, or independent providers you’re collaborating with. You control what each one can access and which of your offices they can work from. Everything logged, everything reversible.

  • Invite staff providers or outside collaborators under one parent practice
  • CHMD verifies every invited provider’s credentials before they go live - your reputation stays protected
  • Set exactly what each provider can do - appointments, prescriptions, billing, or specific subsets of each
  • Lock providers to the office locations where they actually practice
  • Revoke access or change permissions instantly - no IT ticket required
  • Full audit trail so you always know who invited whom and what they changed

Prescription Hub

Approve prescriptions with confidence, not paperwork.

When a patient needs a prescription you’re comfortable issuing, the Rx Hub makes it fast, compliant, and properly documented. Review the patient, sign your decision, and CHMD generates a secure purchase link tied to that specific patient - single-use, time-limited, routed to the pharmacy for fulfillment. You keep a full audit trail from your signature all the way to delivery.

  • Four ways to respond: approve, deny, suggest an alternative, or request more information
  • Your signature and timestamp attached to every decision - compliance baked in
  • Purchase links expire automatically and can only be redeemed by the patient they’re tied to
  • Revoke any link instantly if something changes
  • Keep patient-facing notes separate from your internal thinking
  • The pharmacy sees what they need to fulfill - and nothing more
  • Upload signed prescription files when your workflow requires it

Dual-Sided Reviews

A reputation you build - and a private warning system among clinicians.

Reviews on CHMD work both ways. Patients review you publicly - but every review is tied to a real completed visit, and every public comment is AI-sanitized for HIPAA safety before it goes live. Then there’s the other side: clinicians can privately flag patients to each other. No-shows, drug-seeking, safety concerns - shared confidentially with the provider network, invisible to the patient, never public.

Patient → Provider reviews (public)
  • 1–5 stars overall, plus breakdowns for wait time, communication, professionalism, and value
  • AI-sanitized before publication so PHI doesn’t leak into a public review
  • Every review must be tied to a completed visit - no drive-by one-star attacks from strangers
  • Respond publicly to any review with a threaded reply
  • Helpful voting, report flagging, and CHMD moderation on anything out of line
Provider → Patient notes (private)
  • Rate the encounter from your side: punctuality, communication, compliance, respectfulness
  • Would-see-again and no-show flags help peers triage their own bookings
  • Your written notes are AI-sanitized and shared only within the clinician network - never with the patient
  • Serious concerns - safety threats, fraud, doctor-shopping, threatening behavior - escalate to CHMD staff only
  • Color-coded tags let you categorize an encounter at a glance

MD Connections & Attribution

Partner with wellness pros. Share revenue fairly.

The best patient outcomes often start with a trusted wellness professional - a health coach, nutritionist, or functional medicine practitioner who refers to you for prescriptive care. CHMD turns that relationship into something documented, measurable, and fair: signed agreements, clear revenue splits, and a ledger that tracks every dollar owed in both directions.

  • Formal partnership records - request, accept, decline, or end a connection, all on both sides
  • Four agreement styles: referral split, revenue share, flat fee, or custom terms you define together
  • Apply agreements to prescription checkouts, office visits, storefront sales, or everything at once
  • Pause or turn off attribution on any partnership at any time
  • Shared notes, private notes, and pinned reference items per partner
  • Secure in-platform messaging with attachment support - no personal texts or emails needed

  • Build unlimited pages with custom content, imagery, and search-friendly details
  • Full version history on every page - roll back to any earlier draft whenever you need to
  • Embed images, video, YouTube, Vimeo, or any third-party content
  • Custom navigation menus with nested items, icons, and badges
  • Draft in private, publish when you’re ready, hide individual pages from search

Frequently asked

What LPs want to know before applying

Is ConnectHealthMD free for Licensed Providers?
Yes. Applying is free and the core features are free forever - your MD Network listing, telemedicine and in-person appointments, the prescription hub, reviews, and partnerships with wellness pros. Paid tiers are optional and unlock things like additional office locations, staff seats, advanced analytics, and priority support.
What credentials do I need to apply as an LP?
A valid NPI is required for every LP application. You'll also submit your state license number and specialty. If you prescribe controlled substances, your DEA number is required and verified. Additional documents (board certifications, etc.) can be attached to your application during submission.
Do I need a DEA number to join?
No. DEA is optional and only relevant if you prescribe controlled substances. Plenty of LP specialties don't need one. NPI is the only credential that's always required.
How long does the approval process take?
A few business days in most cases. A real person on the CHMD team personally reviews every application - verifying your NPI against federal records, confirming your state license is active, and checking your DEA if you prescribe. You’ll see your status update as we go, and we’ll reach out directly if we need anything else.
Can I invite my practice staff under one parent account?
Absolutely. If you own the practice, you can invite other clinicians to work under your account - whether they’re on your staff or independent collaborators you work with. Every invited provider has their credentials verified by CHMD before going live, and you decide what each one can do and which of your office locations they can practice from. Revoke or adjust permissions any time.
How does prescription approval actually work?
You review the patient, choose how to respond - approve, deny, suggest an alternative, or ask for more information - and sign your decision with your name and a timestamp. CHMD generates a secure purchase link tied specifically to that patient, with a maximum number of uses and an automatic expiration. The pharmacy handling fulfillment sees only what they need to ship. You get a complete audit trail from your signature to the patient’s doorstep.
How do the dual-sided patient reviews work?
Patients can rate you publicly - 1 to 5 stars plus ratings on wait time, communication, and value - but only after a real completed visit. Every review is AI-sanitized before publication so patient health information can’t accidentally leak. On your side, you can share private notes about patients with the clinician network - flagging no-shows, non-compliance, or concerning behavior - that are never visible to the patient. Serious safety concerns route directly to CHMD staff.
Can I accept insurance through CHMD?
Yes. Patients submit their full insurance information when they book - including photos of their card - so you and your staff aren’t collecting it at the front desk. Everything sits in a clean verification queue with a clear status on each one, so nothing gets lost. You can also configure which carriers each of your office locations accepts, so patients only book where their plan is actually in-network.
How do partnerships with wellness pros actually work?
A wellness professional you trust refers a patient to you for care that requires your license. CHMD formalizes that relationship: a documented partnership, a signed agreement on how revenue is shared (referral split, revenue share, flat fee, or custom terms), and a ledger that tracks every dollar flowing in both directions. You can apply the agreement to prescription checkouts, office visits, storefront sales, or everything at once. Both parties see every change; both parties sign off.
Can I hide from the MD Network temporarily?
Yes. You can pause your visibility in the directory so new patients can’t find you - or stay visible but stop accepting new partnership requests. Either way, existing patients and relationships keep working normally. Both switches are instant and reversible whenever you’re ready to come back.
What happens to my data if I leave CHMD?
You can close your account at any time. We retain only what we're legally required to keep (transaction records, tax documents, compliance logs for prescriptions) and delete the rest according to our data retention policy. You'll be able to export your patient list, appointment history, and content before closing.
